New Patient Registration

Registering as a New Patient

To register as a patient please ask at reception for details or complete the New Patient Registration Form and email to yoxall.healthcentre@nhs.net.

 We will need to see valid identification when registering with the surgery please. Newly registered patients will be invited for a check-up with our Health Care Assistant, once the required forms have been received.

If you take regular medication you will need to book a routine appointment with a doctor before you can be issued with a prescription.

Medical treatment is available from the date of registration. Please contact reception for further information.
